We'll then ask you to complete a Medical Information Form so that we can better understand your accommodation needs. There is a section to be completed by your GP or medical practitioner, and a section for you to fill out. It's important to give us as much detail as possible so we can match you to a suitable room. We also request that you complete and return this information as soon as possible. This is so we have enough time to process your information and make any necessary adjustments needed. If you experience delays or difficulties completing the GP section, please contact us at for advice.
Once we have received your information, the Accommodation Team will review this. If there is any information missing, we may contact you to ask you to fully complete the form. It may also be necessary for a member of the team to organise a virtual meeting with you to discuss the information received. This is usually when specialist equipment or accommodation for a carer has been requested.
We'll then review your form and, if required, identify a room suitable for your needs. While we will make every effort to place you in your preferred room or location, we cannot guarantee it.
